The identification and academic information you provide on this required supplemental form is used by the School of Pharmacy administrative staff to process your application (in addition to the required online PharmCAS application located at http://www.pharmcas.org.) After you complete the online portion of this application, a printable summary page will be displayed. * indicates a required field.
You will need the following items BEFORE completing this online application:
- 1. Personal information
- 2. Check for $60 supplemental application fee and Signature Page: Microsoft Word format | Adobe Acrobat format
- 3. Your PharmCAS ID Number generated from the online PharmCAS application program located at http://www.pharmcas.org
- 4. Written essay information (may be copy and pasted from Word or other
word processing software.) The essay is a statement of purpose that should address the following
areas: Please describe an academic challenge that you faced during your high school or college
education. Please include how you dealt with the challenge and the lesson learned from it, if any.
- 5. List of schools, organizations, community services, leadership
positions and outside interests.
- 6. Letters of Recommendation must be submitted through PharmCAS directly. Follow their instructions closely, http://www.pharmcas.org/applicants/eval.htm.
