UC San Diego Supplemental Application
For Students Entering Fall 2018
You must start your PHARMCAS application and have a PHARMCAS ID number before beginning this supplemental application.
The identification and academic information you provide on this required supplemental form is used by the UCSD School of Pharmacy admissions staff to process your application (in addition to the required online PharmCAS application). Please note that the non-refundable supplemental application fee is $105 and online payment is due upon completion, unless you are requesting a fee waiver. Applicants who submit electronic payment AND submit a fee waiver will not have their payment refunded should the fee waiver be approved. You should wait for the results of your fee waiver request before paying. * indicates a required field.
Supplemental Application Login Instructions
First Time Applicants
(Defined as an applicant who has not logged on for the Fall 2018 admissions cycle. Prior year's applicants are considered first time applicants in this admissions cycle.)
Check the New Application check box, and then enter your email address in the email field. Be sure to use a valid email address in the email address field. This will be the email address that will be used to contact you. You will be assigned a password: Please remember this password as it will allow you to edit your application later.
Applicants Returning to Complete or Modify Their Supplemental Application
Enter your email address in the email address field and your password in the password field. Do not check the New Application check box. You only have to enter your password in the top password field.
You may return to complete or modify your application until the deadline. When you are ready to submit your final version, click Final Submission. After that, you will not be able to make any further modifications to your application.
