Partners in Medication Therapy (PMT), an innovative venture of the UC San Diego Skaggs School of Pharmacy and Pharmaceutical Sciences, offers Medication Therapy Management (MTM) services to external clients such as self-insured employers, medical groups, and health plans.

MTM services focus on optimizing medication usage to improve patient outcomes, enhance patient knowledge, and manage costs for chronic conditions such as diabetes, asthma, and cardiovascular disease, among others. Clinical pharmacy faculty from Skaggs School of Pharmacy are involved in PMT as MTM providers, consultants to community partners, and developers of new care models.

The Skaggs School of Pharmacy is uniquely qualified to offer this new program because its mission is based on delivering best practices of care as well as developing innovative care models, and its clinical faculty are licensed and credentialed in many pharmacy specialty areas. Among its goals are to achieve optimal clinical outcomes and train future generations of pharmacists. Since the Skaggs School of Pharmacy is an integral part of health sciences education and health-care delivery in an academic institution, it is not tied to specific pharmacies.
